Support Coordination is a capacity building service funded by the National Disability Insurance Agency to assist participants and their families to understand, implement, and manage their NDIS Plan. As each participant is an individual with their own unique needs, Support Coordination will be funded based on your support requirements.
If you do not currently have Support Coordination funding and are in need of support, please still contact us!
Connection Avenue works with participants on the National Disability Insurance Scheme to provide Support Coordination and Recovery Coaching, of all ages and abilities. We have experience with children, adults and mature aged participants with physical, intellectual, behavioural and psychosocial disabilities.

Connection Avenue has built lasting connections with providers local to the Wide Bay area, spanning Gympie, Maryborough, Hervey Bay, Childers and up to Bundaberg, and specialises in servicing this region. We recognise that quality and compassionate Support Coordination is invaluable however, and welcome participants outside of this area to connect, to discuss how we can work together.
No, Support Coordination is funded in addition to other supports in your plan. This means that you have the comfort of Support Coordination to help you navigate your NDIS Plan, without using funding from other supports.
Psychosocial Recovery Coaches are trained professionals who work alongside you to:
Understand your recovery goals: They help you identify and define your unique recovery aspirations.
Develop a recovery plan: They collaborate with you to create a personalised plan that outlines your recovery pathway and strategies.
Provide support and guidance: They offer ongoing support, encouragement, and practical assistance as you navigate challenges and celebrate successes.
Connect you with resources: They help you access relevant NDIS services, community supports, and other resources that can aid your recovery.
How can a Psychosocial Recovery Coach help me?
Improved mental well-being: Coaches provide a safe and supportive space for you to explore your recovery journey and develop coping mechanisms.
Reduced symptoms: They assist you in managing symptoms of mental health conditions and promoting overall well-being.
Increased self-esteem and confidence: Coaches help you build self-belief, resilience, and a sense of purpose.
Enhanced social connections: They facilitate your engagement with community resources and support networks, fostering social connections.
With the support of a Psychosocial Recovery Coach, you can take ownership of your recovery, build resilience, and achieve your well-being goals.
What is funded in your NDIS Plan, and what can be paid for from your NDIS Plan are two very different things! If you do not have Support Coordination allocated in your NDIS Plan, please contact us to discuss options that may be available to you, for support through your plan.
